Insurance and Billing Procedures

Park Physical Therapy Associates, Inc., is Medicare Certifiedand a recognized provider with virtually all PPO, HMO, POS, and Workers Compensation plans. As a courtesy, we will bill through your primary insurance carrier. If you have secondary coverage, we ask that you submit your claim directly to that carrier for your remaining reimbursement.

Please remember, your insurance coverage is a contract between you and your insurance company. If a payment is delayed, we need your involvement in receiving payment for services rendered. If we do not receive payment in a timely fashion, we will ask you for a payment of the balance of your account. Co-payments or 20% responsibility is to be paid at the time of each visit. Please be aware that all insurance companies have different policies and procedures on reimbursement so it is important that you know your coverage and abide by their policies.

If your insurance coverage requires authorization by your referring physician, make sure your visits are pre-authorized prior to your physical therapy sessions. Note that by law, physical therapy can only be rendered with a valid prescription signed by your referring physician.
